Central Registration Information...

Universal Pre-K 2009-2010...
Informational letters have been sent out to families in the Sweet Home School District for the 2009-2010 Universal Pre-K school year. If you live in the Sweet Home School District and haven’t received a letter and are interested in sending your child to Universal Pre-K, please call Elaine Skalski at 250-1414. Any child who will reach their 4th birthday on or before December 1, 2009 is eligible to attend. Pre-K registration will take place in April. Registration packets will be mailed to parents in March.





Kindergarten 2009-2010...
Letters  will be sent to parents in January regarding Kindergarten registration. Children who will be 5 years old by December 1, 2009, are eligible.

Please note that if your child is currently attending Sweet Home's Universal Pre-K program, you will NOT need to register your child for Kindergarten. Your child will be automatically transferred to your neighborhood elementary school. The only forms you will receive and need to complete are updated physical, immunizations and a dental certificate. These forms are due 30 days prior to the new school year.

Children who are not currently enrolled in our Universal PreK need to be registered for the 2009-2010 school year. Parents should have received registration forms along with a letter notifying you of the dates of registration for your child's elementary school.  You must schedule an appointment with Central Registration at 250-1600.  If you have not received a letter or registration packet or are not sure which elementary school your child will attend, please feel free to contact Central Registration.  

The designated registration dates for each building are below.


If you are unable to attend during your child’s school’s designated time, please call for an alternative appointment.  These will be held after March 1st but will not delay your child’s registration process in any way.

All Registrations take place at our Central Registration office located at 1901 Sweet Home Road, not at the Elementary buildings.

Glendale Elementary:  February 2,3,4                                   
Heritage Heights Elementary: February 25, 26, 27                                
Maplemere Elementary:   February 12, 13, 23               
Willow Ridge Elementary: February 9, 10,11





Moved Within the District??...                                                  
If you have recently moved within the district you will need to contact the Central Registration Office to change your student’s information. You will need to fill out a change of address form and provide Central Registration with a copy of your new lease or mortgage and a current utility bill. Central Registration is located at 1901 Sweet Home Road (attached to the High School) and can be reached by calling 250-1600.New to Sweet Home?  Here is all the information you need to get your child (ren) registered for the 2009-2010 school year.





New to Sweet Home Central School District??...
The Sweet Home District has a centralized registration process for your convenience. Our Central Registration Office is located at our Family Support Center, 1901 Sweet Home Road, adjacent to Sweet Home High School. You need to obtain a registration packet from us for each child you are registering. Registration packets are located at our office or in each Elementary School’s Main office, Middle School Student Services and High School Student Services.

In addition to the registration packet, you must provide the following information: original birth certificate, up to date immunization record, proof of physical within 12 months of September 3, 2008, academic record/transcript from previous school, picture ID of parent and proof of residency in the district.

You must call the Central Registration Office to make an appointment to turn in your child’s registration paperwork. Our number is 250-1600.

In order to register a child for school, the biological parent or court-appointed legal guardian must have legal residence in the Sweet Home District. It is illegal for children to attend school where the parent or legal guardian does not reside.                                                                                                   

The following proofs of residency are necessary:
1.) Proof of the parent/guardian’s residence. The accepted documentation is as follows:
For Homeowners:                                                         
· A mortgage statement or                                               
· School, Town or County Tax Bill or                                    
· Homeowner’s insurance statement or                                    
· Purchase papers for a home

For those renting or leasing a house/duplex/apartment:          
· Current lease or rental agreement. This must be in effect at the time of registration.                                   
· If you are on month to month or have no current lease then you must provide the following:            
· A notarized statement from the landlord that the parent/guardian and the children are residing at the property, including landlord’s phone number AND                 
· Proof of homeownership from the landlord

If you are not the primary owner or renter of the property:     
· Request from the landlord that your name be put on a new lease or                                                     
· Obtain a notarized statement from the landlord acknowledging that he/she is aware you are living at that property AND the current lease of the primary renter AND provide a car insurance bill, pay stub or bank statement addressed to you at that address.                          
· Obtain a notarized statement from the homeowner that you are living with them and then the homeowner must produce proof of ownership of the house. You must then provide your own proof that this is your legal residence in the form of car registration, car insurance, and bank statement or pay stub.

2.) Second proof of residence is a utility bill. If renting a property, the
utility bill must be in the renter’s name.
