STUDENT REQUIREMENTS FOR SCHOOL ENTRY:
IMMUNIZATIONS, HEALTH CERTIFICATES AND TESTING
Proof of immunization, history of disease, or serology must be presented at registration to a school according to our New York State school health laws. Without proper documentation, students are not officially registered and will not be admitted to school.
The Following Are Required For Your Child To Attend School:
Diphtheria, Pertussis*, Tetanus* (DTP, Dtap, Td); Polio
Three doses are required of:diphtheria and polio (any combination of OPV or eIPV). [*All children born on or after January1, 2005 must be immunized against pertussis and tetanus.]
Tdap is required for those born on or after January 1, 1994 and entering grades 6, 7, 8, & 9 or a comparable age-level special education school or program in the 2010-2011 school year. This requirement is in addition to any previous booster for kindergarten entrance. Documentation of Tdap vaccine must be submitted before September school entry.
Hepatitis B
Three doses are required.[Recombivax HB 10 mcg for two dose schedule, only for adolescents 11-15 years of age.]
Measles, Mumps, Rubella (MMR)
Two doses are required of rubeola(measles), and one dose of mumps and rubella (German measles). MMR vaccine is preferred for measles, mumps and rubella. The first dose of measles, mumps and rubella (MMR) should be given no more than four days prior to the first birthday. The second dose should be given after the age of 15 months and preferably between 4 and 6 years of age. The minimum interval between immunizations should be no less than 28 days.
Varicella (Chickenpox)
All children need one dose of varicella (chickenpox) for pre-k entry. Students born on or after January 1,1998 need one dose for K-12 entry. Students born on or after January 1, 1994,and who enroll in grades 6, 7, 8, 9, 10& 11 at the beginning of the 2010-2011 school year must be immunized against varicella. Varicella should be given no more than four days prior to the first birthday. (It is recommended persons 13 years and older receive two doses of varicella for full immunity.)
Haemophilus Influenza Type B
Three doses are required for pre-k students given before 15 months or one dose on or after 15 months before5 years.
Physical Examination (Health) Certificate
Physical examinations are required in accordance with New York State Education Department School Health Law (Section903) for all new entrants to the district regardless of grade level; students in Grades Pre-K, K, 2, 4, 7, 10; students participating in sports or who need a working permit; and for Special Education students who also require a neurological screening upon initial placement and every three years as needed.
Dental Health Certificate
School districts are required to request a dental health certificate prior to entering Pre-K, Kindergarten, 2nd Grade, 4th Grade, 7thGrade, and 10th Grade and upon new registration to school.
Lead for Preschool Entry
Proof of lead testing is required and' will be requested at registration.
New York State regulations require medical providers to test children for lead at age one year and again at age two years. They must also assess children for lead risk at each well-child visit, and test children up to age six years if a risk for lead poisoning is found.
Please Note- Immunizations can be waived if there is written proof from the student's health care provider of the student having measles, mumps or varicella. Proof of immunization,history of disease, or serology (blood test) must be shown for all immunizations, except rubella can only be verified by serological evidence and not by a diagnosed rash or health care provider letter. Religious or medical exemptions for any immunization must be submitted to school officials in writing.
